Overdosage

Overdosage incidents have not been reported during clinical studies involving Alprostadil. However, in the rare event of an intracavernous overdose, it is crucial that the patient is monitored by a healthcare professional until any systemic effects subside or until the erection returns to a non-erect state (penile detumescence). If symptoms such as low blood pressure or other adverse reactions appear, appropriate symptomatic treatment should be administered promptly. In clinical studies involving 1,861 patients over an 18-month period, prolonged erections lasting between 4 to 6 hours were reported in approximately 4% of cases. A smaller percentage (0.4%) experienced priapism—an erection lasting more than 6 hours—which can result in long-term complications if not treated immediately. Medical attention should be sought without delay if an erection persists beyond 4 hours, as untreated priapism may lead to permanent penile damage and loss of sexual function.

To reduce the likelihood of such adverse effects, Alprostadil should be carefully titrated to the lowest effective dose. Patients with conditions that increase the risk of priapism—such as sickle cell anemia, multiple myeloma, or leukemia—should not use this medication.
